If you are a new patient, please take a minute to fill out our new patient paperwork and liability waiver and bring with you to your first appointment. Doing so will help us maximize our time together. If you forget, no worries, there are extra copies located on the clipboards at the front office. We look forward to working with you soon. What type of payments do you accept?
We accept cash, check, and credit card payments at this time. Payment is due in full on the day of your service.
Can you bill my insurance?
In order to provide you with the best possible care and spend the proper amount of time with you we are unable to accept insurance payment at this time for physical therapy. However, we are happy to provide you with all the necessary documentation for you to submit your claim to your insurance directly. We are an out of network provider and any payment by insurance will go directly to you. Please contact your insurance provider to find out what your out of network benefits are. Click on the checklist button to the right for a helpful guide of questions to ask when you call.
Can I use a HSA or FSA to pay for my exercise classes?
Please check with your plan but often all or a portion is available to submit using your receipt.
Why is there a package discount?
We have chosen to offer a package discount because it greatly reduces our administrative time to manage individual accounts. Customers receive a 5-10% discount when buying services in a package. All packages expire 6 months from the first date of service.
